Executive Summary

This report identifies 15 critical problems and 12 medium-priority issues across the Kaizen OS monorepo. The analysis reveals gaps in governance enforcement, incomplete service implementations, and missing critical infrastructure components that could compromise system integrity and operational safety.

Risk Level: 🔴 HIGH - Multiple blocking issues prevent production readiness


Tier 1: CRITICAL PROBLEMS (Blocking Production)

1. ❌ CODEOWNERS References Non-Existent GitHub Teams

Problem: .github/CODEOWNERS references teams like @AUREA, @ATLAS, @ZEUS, @HERMES, @EVE, @ECHO that likely don't exist as GitHub teams.

**Impact:** 
- Pull request reviews bypassed (GitHub can't find reviewers)

- Violates Trinity Seal requirements

**Fix Required:**
- Create GitHub teams: `kaizencycle/aurea`, `kaizencycle/atlas`, etc.
- OR replace with individual maintainer GitHub usernames
- Test CODEOWNERS enforcement with a test PR

**Priority:** 🔴 P0 (BLOCKING)

### 2. ❌ Epoch Burn Service Not Implemented

**Problem:** `configs/integrity_units.yaml` defines epoch burn configuration, but no standalone service exists to execute it.

**Evidence:**
- Config exists: `configs/integrity_units.yaml` (lines 24-32)
- Service directory missing: `services/epoch-burn/` does not exist
- Health endpoint hardcodes: `days_until_burn: 45` (TODO comment)

**Impact:**
- Integrity inflation risk (no decay mechanism)
- Economic model incomplete
- Violates foundational blueprint specification

**Fix Required:**
- Create `services/epoch-burn/` with:
  - Cron job runner (every 90 days)
  - Idle shard detection (>30 days)
  - Decay calculation (0.5% per epoch)
  - Reabsorption to integrity_pool
  - Signed attestation emission
  - Integration with civic-ledger

**Priority:** 🔴 P0 (BLOCKING)

### 3. ❌ Missing Attestation Endpoints (/attest/mint, /attest/burn)

**Problem:** Civic Ledger service lacks critical attestation endpoints for value creation/destruction.

**Evidence:**
- `services/civic-ledger/src/app.ts` shows no `/attest/*` routes
- Only conversion endpoints exist: `/convert/shards-to-credits`, `/convert/credits-to-shards`
- Gatekeeper service references mint/burn but calls stub endpoints

**Impact:**
- No proof-of-integrity for minting/burning
- Cannot enforce dual-signature requirements
- Violates governance model (GI ≥ 0.95 floor not enforced)

**Fix Required:**
- Add `POST /attest/mint` endpoint:
  - Verify GI ≥ 0.950 floor
  - Require dual signature (human + sentinel)
  - Generate immutable attestation hash
  - Log to ledger with timestamp
- Add `POST /attest/burn` endpoint (same pattern)
- Add integration tests for signature verification

**Priority:** 🔴 P0 (BLOCKING)

### 4. ❌ GI Aggregator Returns Hardcoded Value

**Problem:** GI endpoint (`/gi`) returns hardcoded `0.999` instead of real calculation.

**Evidence:**

Impact: - All GI gates are bypassed (always returns healthy) - Cannot detect integrity degradation - Violates core system principle (GI ≥ 0.95 enforcement)

Fix Required: - Implement GI aggregator service - Calculate from attestation indexer data - Factor in: Memory (M), Human (H), Integrity (I), Ethics (E) - Formula: GI = 0.25*M + 0.20*H + 0.30*I + 0.25*E - Cache with TTL (5-10 minutes)

Priority: 🔴 P0 (BLOCKING)


5. ❌ Health Endpoint Has TODOs

Problem: /system/health endpoint returns placeholder values with TODO comments.

**Impact:**
- Operations cannot diagnose misconfigurations
- Unit drift detection impossible
- Monitoring/alerting unreliable

**Fix Required:**
- Calculate `current_epoch` from ledger genesis timestamp
- Calculate `days_until_burn` from last burn event
- Wire to actual GI aggregator (see Problem #4)

**Priority:** 🟠 P1 (HIGH)

### 6. ❌ Missing .env.example for Civic Ledger Service

**Problem:** No environment variable template exists for `services/civic-ledger/`.

**Evidence:**
- Root `env.example` exists but is generic
- `services/civic-ledger/.env.example` does not exist
- Service reads: `PORT`, `EPOCH_BURN_ENABLED`, `GI_FLOOR_WARN`, `GI_FLOOR_HALT`

**Impact:**
- Developers cannot configure service locally
- "Works on my machine" errors
- Deployment misconfigurations

**Fix Required:**
- Create `services/civic-ledger/.env.example` with:
  - `PORT=3000`
  - `EPOCH_BURN_ENABLED=false`
  - `EPOCH_LENGTH_DAYS=90`
  - `DECAY_PERCENTAGE=0.5`
  - `GI_FLOOR_WARN=0.950`
  - `GI_FLOOR_HALT=0.900`
  - `GI_AGGREGATOR_URL=http://localhost:3001`

### 7. ❌ CI/CD May Not Enforce All Gates

**Problem:** GitHub Actions workflows exist but may allow failures to pass.

**Evidence:**

Problem: Monorepo has no per-package version management.

Impact: - Consumers cannot pin safe versions - Breaking changes not tracked - No release automation

Fix Required: - Install @changesets/cli - Configure changeset workflow - Add GitHub Action for version bumps - Generate per-package CHANGELOG.md

Priority: 🟡 P2 (MEDIUM)


Tier 2: ARCHITECTURAL PROBLEMS

9. ⚠️ Monorepo Scaling Risk

Problem: 43 packages/apps in single repo without clear boundaries.

11. ⚠️ Test Coverage Unknown

Problem: No test coverage reporting or minimum thresholds.

12. ⚠️ Security Scanning Not Automated

Problem: Security scan exists but may not block merges.

**Problem:** Gatekeeper service calls `mint_gic` but civic-ledger has no such endpoint.

Impact: - Maintenance burden - Bug fixes must be applied twice - Risk of divergence

15. ⚠️ No Database Migration Strategy

Problem: Civic Ledger service has no database schema or migrations.

Testing Checklist

Before considering production-ready:

  • CODEOWNERS enforcement tested (create test PR)
  • CI gates block on failure (test with failing code)
  • Epoch burn service runs successfully
  • Attestation endpoints verify signatures correctly
  • GI aggregator returns real values (not hardcoded)
  • Health endpoint shows accurate epoch/burn data
  • Database migrations run cleanly
  • Test coverage >80% for core packages
  • Security scan blocks vulnerable dependencies
  • Versioning workflow generates changelogs

Conclusion

Kaizen OS has a solid foundation but requires immediate attention to critical governance and infrastructure gaps before production deployment. The most urgent issues are:

  1. CODEOWNERS not enforcing reviews (governance bypass)
  2. Missing epoch burn service (economic model incomplete)
  3. Hardcoded GI values (integrity checks bypassed)
  4. Missing attestation endpoints (no proof-of-integrity)

Recommendation: Address P0 issues within 1 week, P1 issues within 2 weeks, before accepting external contributors or public deployments.
